At the beginning of theIVF ROSI technique, round spermatids, which are not capable of fertilising the egg on their own, are activated by special laboratory methods. The activated spermatids are combined with the egg to form an embryo. In the ROSI technique, sperm activation is achieved usingchemical methods or electric current. This activation is carried out in the process of merging with the egg and the egg is fertilised.
Sperm production is a complex and long process. Spermatogonium, the sperm stem cells, start with 46 chromosomes and spermatocytes form spermatids as a result of meiosis. Round spermatids are round cells that do not have a tail structure in the initial form. These cells are characterised by having the same number of chromosomes as sperm. Later, they start to grow a tail, which they are called elongated spermatids, and become mature sperm cells. Without any intervention, round spermatids and elongated spermatids cannot fertilise the egg.
Sperm Production
ForROSI treatment, the spouses first undergo a urological and gynaecological examination. If there is no obstacle to treatment, treatment is started with the ROSI technique. Like the standard IVF treatment, the ROSI technique starts on the second day of menstruation with the woman taking egg stimulant medication. On the day of egg collection, the man undergoes micro TESE. The sperm cells collected afterwards are analysed in the laboratory. As a result of the examination performed by experienced embryologists, if mature sperms are identified, these sperms are used. If there are no mature spermatids, round and elongated spermatids are collected. Some of the collected spermatids can be frozen. Oocytes taken from the woman are stimulated by piezoelectric method. Withthe microinjection method, round spermatids are placed into the egg and fertilisation is performed after waiting 24 hours. Embryo development is monitored for 3-5 days and the best quality embryo is transferred to the mother's fertility bed.
ROSI Technique
ROSI technique is a method applied to prospective fathers who have spermatids but do not have mature sperm in semen. Before ROSI treatment, hormone values, testicular ultrasound and genetic tests are evaluated. In some cases, hormone or drug treatment may be applied before the treatment is started.
